DENTAL ELEMENTS PRIVACY POLICY AND CONSENT

We are committed to protecting the privacy of our patient’s personal information and to utilizing all personal information in a responsible and professional manner. Our Privacy Policy outlines the personal information that we collect, use and disclose. In addition to the circumstances described in this policy, we also collect, use and disclose personal information when permitted or required by law.

If we are ever considering selling all or part of this dental practice, qualified potential purchasers may be granted access as part of the due diligence process to patient information in order to verify information important to the potential sale. If this occurs, we will take steps to ensure that the prospective purchaser safeguards all personal information.

All Dentists are regulated by the College of Dental Surgeons of Alberta (CDSA), which may inspect our records and interview our staff as part of its regulatory activities in the public interest.

Patient’s Medical Information is collected and used for the purpose of diagnosing dental conditions and providing dental treatment. We collect information from our patients about their health history, their family health history, physical condition and previous dental treatments. This information may be disclosed to other Dentists, Dental Specialists, and other Health Care Professionals for the following:

  • If we have referred the patient for treatment or a second opinion
  • If the patient has been referred to our office for treatment or a second opinion
  • To third party health benefit providers and insurance companies, where a claim for reimbursement (partial or full payment), has been submitted on the patient’s behalf.

Contact Information is collected from our patients such as name, address, daytime phone number and email address for the purposes of starting new and/or updating computer patient profiles. This is also used to send reminders to patients regarding due or overdue examinations, cleanings, treatment, appointment confirmations and dental statements. Contact information may also be disclosed to third party health benefit providers, insurance companies (if an insurance claim has been submitted for reimbursement or payment) and Collection Agencies when necessary.

Financial information may be collected in order to make arrangements for the payment of dental services.

To comply with the Canadian Anti-Spam Legislation that came into effect July 01, 2014, we are committed to never sending spam emails and sending information and communication via email for our patient’s convenience. Patients may opt out of receiving email and withdraw consent at any time.
